CPT® sections, symbols & the surgical package

15 min read

Category I CPT® codes are five digits arranged in six sections: Evaluation and Management (99202–99499), Anesthesia (00100–01999), Surgery (10004–69990), Radiology (70010–79999), Pathology and Laboratory (80047–89398) and Medicine (90281–99607). Category II codes end in F (performance measurement, optional), Category III codes end in T (emerging technology, reported instead of an unlisted code when available). The global surgical package bundles the operation, local anesthesia, and typical follow-up care.

  • ✓Symbols: ● new code, ▲ revised, + add-on code (never reported alone, modifier 51 exempt), ⊘ modifier 51 exempt, ★ telemedicine.
  • ✓Major surgery has a 90-day global period; minor procedures 0 or 10 days.
  • ✓Separate procedure designation: bundled when performed with a related, more extensive procedure; reportable when performed alone.
  • ✓Unlisted codes require a special report describing the nature, extent and time of the service.
  • ✓Read the section guidelines and all parenthetical notes before assigning a code — they are testable and binding.
